We offer 24-Hour Supported Community Living (SCL), Consumer Directed Attendant Care (CDAC), Habilitation Services and Respite Services. We also run a unique Host Home program that allows individuals with disabilities to move in with host home providers to receive care and support and to enjoy community life in the best possible way.
24-HR SCL
We provide individuals the opportunity to live as independently as possible. Our team of experienced staff offer individuals the opportunity to grow and work toward greater independence. Whether providing covered services like support with safety, housekeeping, finances, communication, medical appointments, or supervision, we ensure the individual is at the center, and that the focus is to increase the individual’s quality of life by ensuring the individual lives, learns, work and enjoy life in the most integrative way, and lead a life that is meaningful and fulfilling in every aspect.
CDAC Services
We recognize the need of some individuals to stay in the comfort of their own homes while receiving services. We offer individuals the opportunity to stay in their own home while their needs are being met. We ensure that all services are provided according to the individualized needs and preferences of the person as identified in the service plan. We emphasize respect for the individual’s dignity and independence and ensure that the individual is physically comfortable, safe, and supported in the most practical ways
Host Home Program
Our professional team of host home providers open their home to individuals with disabilities. Our host homes provide individuals with disabilities the ability to accomplish their dreams, to learn, and to experience new adventures. Presently, the bulk of our host homes are in the Iowa metro area which includes Des Moines, West Des Moines, Ankeny, Altoona, Grimes, Urbandale, Johnston, Waukee, and Clive. We continue to expand our host home program and envisage future presence in other areas in Iowa. Respite Services
Caring for an aged, sick, or disabled family member is a difficult task that no one is prepared to undertake alone. Finding respite care services might give you a much-needed break. We provide in-home respite services, which includes offering care and support, and carrying out activities to maintain personal care and other activities of living for people in their own homes or in other community settings like Adult Day Centers, Prevocational Centers, Group Supported Environments and Residential Care Centers. Our patient, companionate, diversified and highly trained team members provide quality individualized support while promoting the independence and wellbeing of the individual.
